Upon entering the office, we have some brief paperwork for you to fill out. The paperwork provides us with general information about yourself and your condition. To expedite the paperwork process, we have attached the paperwork below. Please fill out the new patient paperwork to the best of your ability.

Consultation and Examination

Once the paperwork has been filled out, Dr. Gill will go over your health concerns with you. In order to determine the correct diagnosis, Dr. Gill will ask you various questions related to your condition. Next, a number of specialized tests will be performed to determine which areas are affected and to what extent.

X-ray Studies

Your specific condition may require us to take x-rays to either rule out more serious conditions or assist us in developing the most effective treatment plan for you. X-rays are done in office to reduce the amount of time lapse between diagnosis and treatment.

Same Day Treatment

Once finished with the examination and all appropriate studies, patients will generally be provided with their first treatment during the visit. Treatments include: Spinal Adjustments, Physical Therapy and Muscle Testing. Since each patient has their own custom treatment plan, this may or may not include all forms of therapy.

Home Instructions

Prior to leaving, patients will be given instructions on certain activities or procedures to be conducted at home. This may include ice or heat application instructions, avoidance of certain activities or positions, as well as home exercises and/or stretches. Patients who are included in our nutritional treatment plans, will be given specific instructions for their nutrition as well as possible avoidance of certain items.

Scheduling Your Next Appointment

Payment is due at time of rendered services. We accept most insurance and will handle the paperwork from there. We also have very reasonable payment options for those patients that do not have insurance or do not have benefits with their insurance company.

Most of our patients live very busy lives and this is why we will accommodate each patients individual scheduling needs, by providing flexible hours including weekends and two locations to better serve you.
